Let’s get started!

Before you begin, you may need to gather the following items:

• Last year’s tax return (only your income is needed.  We do not need your spouse’s income)
• A list of any patent applications where you are an inventor
• An email address for your patent agent or attorney (optional) so that we may email them your calculation status
• A reference number or docket number that your patent attorney or agent uses to keep track of  your invention (optional)
• About 5 minutes of your time

(note – if your patent will list more than one inventor, each inventor must fill out this form separately)
